Systems of care navigation including federal & state applications for community based social services & supports
Community screenings for: Vision, Cardiac, & Mental health bolstered by Immunization clinics with our public health partners
Mobility Assistance Including: Fuel cards, Applications for donated vehicles, Power wheelchairs / Durable Medical Equipment, Disability modification applications for currently owned vehicles, & Referrals for handicapped accessible vehicles
